Maternal Age: A Major Factor Affecting The Outcome of In Vitro Fertilization

Abstract
Data from 257 cycles in 173 patients treated with gonadotropins for multiple follicle recruitment from April 1988 to May 1990 were analysed to see how maternal age affect the outcome of in vitro fertilization(IVF).
In this study, we excluded the patients who administered GnRH analogues for pituitary suppression, in order to evaluate that how the maternal age works on the efficiency of IVF-ET program stimulated with gonadotropin alone.
We divided all the cycles into three groups according to the maternal age and compared the results. The cancellation rate was significantly higher in patients aged 35 years or more compared with that of younger patients.
The most common cause of cancellation was poor follicular response. Mean plasma estradiol level around the hCG administration day(DO) in patients younger than 35 years of age was significantly higher compared with those of olders. Mean number of
follicles aspirated were significantly different among the three groups. Mean number of oocytes retrieved in the patients aged 40 years or more was significantly lower compared with those of the youngers. No significant differences were found in
fertilization rate and cleavage rate of the oocytes among the three groups. Although there was no difference in embryo transfer(ET) rate per OPU cycles, ET rate per stimulated cycles in the patients aged 34 years or younger was significantly
higher
compared with those of older patients. Mean number of embryos transfered and pregnancy rate were also significantly lower in patients aged 40 years or more compared with those of youngers.
Above results suggest that inspite of the gradual changes in ovarian physiology with the female aging, there was no significant difference in the outcome of IVF before the age of 40 years, however, it was affected after the age of 40 years.
